"What version of Scoop are you running?"
This would help put the answers to the other questions into perspective. This is especially important for some of the things that have gone through some relatively recent improvement (ie HTML in the code, and, oh yeah, the new SAG ;-) )
I also don't really like the phrasing of questions 25 to 27. Well, more accurately, I don't like the use of the word 'never' in them. Every site is going to have a few users that have those issues regardless of what software it's running, so while I'm not a professional surveyor, it seems to me that those questions are going to be skewed towards the 'disagree' end. If you could re-word them to avoid the absoluteness of 'never', much like questions 28-30, I think that would improve the accuracy of the results.
I don't think a scoop survey is a bad idea; however, I think your current set of questions could stand some refinement. Surveys are notoriously difficult to get properly worded and balanced so that the results are an accurate reflection of the people's opinions, rather than what the survey writer believes people's opinions to be.
